  • You may be required to provide the name and phone number of your insurance agent.
  • Your federal tax returns for the previous 2 years. This may only apply if you were self-employed or earn a commission only income.
  • A complete copy of any bankruptcy papers, if you had to file for bankruptcy, as well as the discharge.
  • If you went through a divorce, you may have to supply your divorce decree and separation agreement.
  • Any lease agreements on rental properties.
  • If you supply child support, you may be required to show proof of this.
  • If you have any gaps in employment, you may have to supply a letter of explanation detailing the reasons for your unemployment.
  • If you are a veteran, you may qualify for a VA loan and will need a Certificate of Eligibility.
  • A copy of your homeowners insurance declarations page
